2025 Recognition Application

We confirm that our organization will not operate any 11U or younger teams. By completing this application, we acknowledge that the organization automatically suspends its recognition, if it operates, directly or indirectly, a 11U or younger team at any time. We understand that a suspension of our recognition does not entitle us to a refund of our fees paid for recognition.

The organization shall include the following fees with the filing of this application:
(a) If the organization was not recognized by Baseball Ontario for the 2024 season, payment of $1,000 as a one-time fee; and
(b) Annually, a fee of $300/team for each team that the organization will operate for the 2025 season
We acknowledge that we will not be recognized until our fees are paid, including any outstanding fees from any prior year.

As part of recognition, the organization shall deliver to Baseball Ontario prior to commencing its 2025 season the following documents:
1. A roster for each team which the organization is operating for the 2025 season, which roster shall be updated to reflect any changes; and
2. An application for insurance through Baseball Ontario at the member rate, or provide evidence of satisfactory liability insurance coverage.
